Facility Use Request Page

Crestwood uses an online facility request program to schedule events and activities. If you have never requested the use of one of our facilites, please click the 'register' button below to create an account (valid email address is required). If you are a returning user, please click the log in button below and use your existing account to submit a new request. All users returning must have current liability insurance on file please log in to update, and new must have insurance information when creating a new group.

Facility Rental Fees:                                                                         

All Gymnasiums: $35.00 per hour + setup/cleanup personnel per hour as outlined below

All Cafeterias: $30.00 per hour + setup/cleanup personnel per hour as outlined below              

All Class and Conference rooms: $10.00 per hour + setup/cleanup personnel per hour as outlined below          

All Athletic Fields and Field House: $30.00 per hour + setup/cleanup personnel per hour as outlined below

Stadium, including track: $50.00 per hour + setup/clean-up personnel per hour as outlined below
*an additional $50.00 per hour may be assessed if stadium lighting is required (totaling $100.00 per hour) + setup/cleanup personnel per hour as outlined below

Parking lots only: $35.00 per lot + setup/cleanup personnel per hour as outlined below

Personnel Costs: (2 hour minimum)

• Personnel charges are added when any of the employees listed below are used outside of their regular work shift, (before or after hours and/or on weekends)
• Personnel are not to be paid directly
• Set-up and/or teardown + clean-up time will apply
• Crestwood personnel must be present at all events occurring outside of normal work shifts (before or after hours and/or on weekends)

Custodial and Maintenance personnel: $45.00 per hour
Cooks: $30.00 per hour

A1 Commercial or profit-making organizations.
A2 Private Social function, from within or outside of the school district
A3 Community organizations from other school districts
A4 Community organizations, outside of the school district, formed for charitable, civic, social, religious, or educational purposes.
A5 Users and/or groups not related to the Crestwood Local School District
A6 Political organizations

B1 Users and/or groups related to The Crestwood Local School District
B2 Meetings of employee associations
B3 Use for voter registration and elections
B4 Departments or agencies of the government
B5 Community organizations, from within the school district, formed for charitable, civic, social, religious, or educational purposes.
B6 Crestwood Local Schools Youth Programs
